Titles
Good points to consider, but I would speculate that use of titles probably varies by markets and market size. Some places are more polite, formal, etc. The important issue seems to be that newsrooms have a policy in a style guide so that they are consistent in how they use them. I thought you were going to ask why people called Dr.Tiller an abortion doctor. He performs abortions, but isn't he an ob-gyn? Orthopedic docs aren't "broken arm doctors" or "hip replacement doctors", are they?

Titles
Stacey, This will label me as one of the “older viewers/listeners,” but I’ve worked in several newsroom and the use of titles varied. Some stations had a policy of full name and title on first reference, then just the last name on second reference. The exception was for elected officials, i.e. Mr. Obama. One newsroom required titles on all references for all newsmakers except sports figures and convicted criminals. I think it makes little difference which style you adopt as long as you are consistent. I am comfortable with Dr. Tiller and Mr. Roeder OR simply Tiller and Roeder…as long as each receives the same treatment. It’s a matter of balance and perception of fairness. However, this “older” news consumer objects to any reference to first names for any newsmakers…even in features. We’re not friends; we’re journalists.

Courtesy titles
Use of titles with members of the police force or the military helps a listener/viewer understand where the person fits in the heirarchy. A general probably (ok, hopefully) has a better overview than a private.
